尊敬的代理及客户：

由于EDI 或MOC 订舱时代理所提供的危险品信息不足以完成危险品订舱及危险品货物的船东申报，因此，再次强调我司对于危险品订舱审核及向船东进行危险品申报都以客户及代理填制的附件格式的DGD FORM 危险品表格为准

（见附件DGD FORM，确保都有公司盖章及手写签名, 无公司盖章及手签不予放舱及船东申报）。

有关危险品表格填制请见以下，但不仅限于此。请代理及客户确保实际出运的危险品及海事申报与向我司订舱时申报的DGD FORM填制的信息保持一致。

特别需要指出，瞒报误报判定的时间点。例如，危险品重箱进港后发生的 DG信息更改视作 mis/un declaration…etc, 因此再次强调有任何危险品货物信息（DGD FORM）的更改请确保在危险品重箱进港前书面通知（邮件+更新的DGD FORM）我司客服人员，

我司客服人员会将更新的危险品再次审核并向船东更新申报。

Misdeclaration ofCargo

E. In the event that misdescription of cargo is found to fall within the followingsituations, either by means of physical cargo inspection by Carrier; byevidence of other supporting documents or by change of declaration from shipperwhen cargo is inCarrier's custody:

- From general cargo(GC) to hazardous cargo (DG) either in dry or refrigerated stowage, or

- Change of hazardouscargo description such as, but not limited to change of class; UN; packinggroup; technical name or concentration percentage, or

- Prohibited/RestrictedCargo, or

- Local Authorityconfirmed cargo smuggling with cargo misdeclaration.

Shipper shall be liablefor the following additional payment prior to the release of cargo:

1. Difference in freightand charges between the actual cargo description and the originally declareddescription.

2. Anadditional liquidated damage to the Carrier at US$10000 per container.

3. If physicalinspection is performed, all costs associated with physical inspection plus anadministrative fee of US$75 per container. Shipper shall further indemnify andhold Carrier harmless from and against all loss, damage, expense, actions andclaims for injury to or death of persons and damage to property arising out ofsuch misdescription.
